Application Form

It is important that all relevant sections of this form are completed in detail, only where this is the case can your application proceed to the next stage.

We require the following documents to commence an application

1. Completed application form – page 11 MUST be physically signed by whoever the fee payer will be either 1 or both parents – digital signatures are not accepted by CSBB
2. A copy of the applicant’s Birth Certificate
3. A copy of the applicant’s passport and visa or Australian Citizenship Certificate (if not born in Australia)
4. A copy of the applicant’s Australian Citizenship Certificate or Australian Passport (if both parents born overseas)
5. A copy of the applicant’s Baptismal and other Sacramental Certificates (catholic or other if applicable)
6. A copy of the applicant’s Immunisation History Statement (available from Medicare online through myGov)
7. A copy of the applicant’s most recent School Report
8. A copy of the applicant’s most recent NAPLAN Report
9. A copy of any Specialist Reports and / or assessments for speech, hearing, psychologist for educational need (as indicated on enrolment form)
10. A copy of Family Court Orders (if applicable)
11. Any other documents you deem relevant to support the application.

Please ensure you complete the checklist on Page 11 and append the required copies with your application.
